• 2x - in the Astral Plane upon entering it at Crèche Y'llek
• 3x - in the Astral Plane upon entering it at the start of Act Three when being ambushed by Githyanki, two on islands floating around the skull, one on the skull island with  Orpheus and the  Emperor on it
• 1x - inside a wooden trunk in a shed north of the potion shop at Wyrm's Crossing
• Sold by various merchants once the party reaches sufficient level, including:
  •  Bareki at Undercity Ruins in the Lower City Sewers
  •  Derryth Bonecloak (after Level 6) in the Underdark
