Blasphemous Acolyte may appear during the final battle Atop the Netherbrain if the party side with the Emperor and a Cleric is present.

The target receives Disadvantage Icon.png Disadvantage on Charisma Ability Checks, and is afflicted with Vulnerability to all damage.

The creature begins to mortify, becoming a festering flesh sack propped up by bones, impairing its Charisma.
